Care Plans

Anyone living with a chronic condition is eligible to have a management plan through Medicare. Management plans are the key to help you to plan to improve your health. It will enable you to take control of your chronic condition and get more out of life. A management plan is an agreement between you, your Doctor and other allied health professionals to set achievable goals in order to improve your health and well being.

As your GP, we would like to offer you this service at our practice. There is no cost to you as it is completely covered by Medicare.

If you require the help of allied health professionals such as dieticians, chiropractor, mental health worker, occupational therapist, physiotherapist, podiatrist, psychologist etc. you will be referred as part of this scheme and generally their services will be provided under Medicare. However in some circumstances you may be charged a gap payment. With your Management Plan in place you can attend five allied health professional services per calendar year.

The Management Plan is not compulsory but we believe that it may help us identify and manage any health needs you may have. Our aim is to enable you to enjoy better health for longer!

If you are interested, please phone the surgery to book an appointment with the nurse who will discuss your health care needs, discuss your goals and write the action plan with you. You will also need to ask the receptionist for an appointment with your usual doctor preferably on the same day. Your GP management plan will be reviewed every 6 months and a new plan will then be arranged every 2 years.

Health Assessments

Over 75 Health Assessments

You may be aware that once you are over 75 you are eligible for a free annual health assessment. This is part of the Federal Government’s initiatives to improve the health and the care available to Australians aged over 75 years.

As your GP, we would like to offer you this service at our practice. There is no cost to you as it is completely covered by Medicare. All it takes is an hour of your time.

This health assessment is an opportunity for your Doctor (with the help of the Practice Nurse!) to take a detailed look at such things as your general health, including sight, hearing, diet, sleep, immunisations and any medication you might be taking. It also enables us to discuss any help you may be receiving at home (e.g. home help, meals on wheels) or whether you need any further assistance.

The health assessment is not compulsory but we believe that it may help us identify and manage any health needs you may have. Our aim is to enable you to enjoy better health for longer!

The health assessment will initially be with our Practice Nurse. This usually takes around 45 minutes or so and then your Doctor will see you straight after for a further discussion. At this time, you can discuss any recommendations to help you maintain or improve your health and to prevent health problems in the future.

45-49 Health Assessments

This surgery is currently undertaking a government initiative targeted for all patients aged 45 to 49 years of age. It is recommended at this age to have this health assessment done, to enable information to be gathered about your health, and in turn provide any appropriate follow up to maintain or improve your health.

The health assessment is for a patient who, in the clinical judgement of the medical practitioner and based on the identification of a specific risk factor, is at risk of developing a chronic disease.

A health assessment at this stage of life can assist patients to make the necessary lifestyle changes to prevent or delay the onset of chronic disease.

The following topics will be discussed during your Health Assessments:

  • lifestyle risk factors, such as smoking, physical inactivity, poor nutrition or alcohol use;

  • biomedical risk factors, such as high cholesterol, high blood pressure, impaired glucose metabolism or excess weight; and

  • a family history of a chronic disease.
